INSTRUCTIONS FOR HOCKETTES 2026/27 TIMESHEET

1. Enable Editing
2. Fill in Employee Name
3. Fill in Week Of: use format mm/dd/yyyy - This will populate the correct dates in Day of Week column

NOTE: ALL AAFSC work weeks begin on Saturday

4. Fill in hours under correct TEAM and on correct date. Please fill in hours to the nearest quarter hour (.25)

NOTE: if it is NOT a team practice then it is probably considered to be TRYOUTS. Adult team will be added later this month.

5. Fill in your rate (yellow box). If not sure of rate contact Lia Forsyth
6. Total will calculate automatically.
7. Sign and date where it says Employee Signature. Date format is mm/dd/yy.
8. CHECK timesheet for accuracy
9. Send completed Timesheet to Lia Forsyth

Accident Report Forms

Please Note: All 3 Forms MUST BE completed in full including: Witness Statement, Rink Guard Incident Report Form, and EITHER the Off-Ice Accident Report OR the On-Ice Accident Report. Everything must be signed by the person(s) filling out the forms. All completed forms should be turned in to the AAFSC Office. Learn more about the Accident Report procedures - HERE.
